The crash occurred at Cloverdale Road and San Pasqual Valley Road about 6 p.m., Officer David Stafford said.
A car driven by an 18-year-old man ran a red light and broadsided the older driver's car, Stafford said.
The 54-year-old man was transported to Palomar Medical Center in Escondido.
Stafford did not say whether the younger driver was injured or whether police cited him.
Alcohol is not suspected in the crash, the officer said.
